Support Coordination & Psychosocial Recovery Coaching
Providing Inclusive and Quality Support Services
to Melbourne and Regional Victoria
At Inclusivity Support Services, we specialise in complex case navigation, providing compassionate, strategic coordination supports that help participants get the most out of their NDIS plans — and their lives.
Our team walks beside individuals with diverse needs, including those with psychosocial disability, high-risk environments, or multiple intersecting barriers. With deep experience, cultural understanding, and trauma-informed care, we help participants access the right services, advocate for their goals, and build long-term independence.
Support Coordination
- Understand your NDIS plan
- Connect with trusted service providers
- Build your capacity to manage supports
- Make informed decisions
- Prepare for plan reviews
- Troubleshoot service gaps or breakdowns
- Stay empowered throughout your NDIS journey
Specialist Support Coordination
For participants with more complex support needs, we offer Specialist Support Coordination that addresses:
- High-level risks or vulnerabilities
- Coordination across multiple stakeholders (e.g. justice, child protection, hospitals)
- Crisis resolution and stabilisation
- Clinical or behavioural support planning
- Goal-focused collaboration and escalation support
Our SSCs work closely with allied health teams, housing services, behavioural practitioners, and legal or medical professionals. The goal is long-term outcomes — not just short-term fixes.
Psychosocial Recovery Coaching
Our Recovery Coaches are trained, trauma-informed, and committed to walking with individuals who live with psychosocial disability. We focus on relationship-building, capacity building, and recovery-focused support tailored to each participant’s personal definition of wellbeing.
We provide:
- Recovery planning co-designed with the participant
- Support with life skills, service navigation, and confidence building
- Collaboration with families, carers, and clinical teams
- Flexible options: choose a coach with lived experience or one with professional expertise
We believe recovery is not about a quick fix — it’s about growing in hope, purpose, and connection.
What is Psychosocial Disability?
Psychosocial disability refers to the functional impact of a mental health condition on a person’s ability to communicate, interact socially, manage daily tasks, or participate in education, employment, and community life.
It may be associated with conditions such as:
- Schizophrenia
- Bipolar disorder
- Severe depression or anxiety
- PTSD or OCD
People living with psychosocial disability may experience challenges across areas like learning, self-care, mobility, or emotional regulation. That’s why individualised, recovery-oriented support is so important.
What Does Recovery Mean under the NDIS?
The NDIS defines recovery as “achieving an optimal state of personal, social, and emotional wellbeing, as defined by each individual, while living with or recovering from a mental health condition.”
Recovery isn’t linear. It’s personal, relational, and grounded in autonomy. Our job as recovery coaches is to offer consistent, empowering support — while helping you build resilience, identity, and meaningful connections.
